The Ontario Health Insurance Plan (OHIP), while extensive in coverage, may not cover all services a patient requests. The patient will be responsible for payment when these services are requested. These fees are set out by the Ontario Medical Association (OMA). You will have the option of either paying for each individual service as you use it, or by paying an annual block fee. Annual Block Fee:

$120 - Per Patient

$210 - Per Couple

 The Following Uninsured Services ARE also covered by the Annual Block Fee:

  • Prescription refills by phone or fax (emergency short term refills at physician’s discretion only)

  • Telephone advice (in emergency situations and only at physician’s discretion)

  • Driver’s Medical Examinations / Third Party Physicals (will be reduced to $75 if annual fee paid)

  • Notes or letters on behalf of patients (sick note / 3rd party form / short forms for work or school / camp form )

  • UIC Disability or maternity certificate

  • Pre-employment certificate of fitness

  • Travel cancellation certificate

  • Certificate of medical status

  • Children's aid medical certificate

  • Massage / orthotics / physiotherapy / chiropractor referral

 The Following Uninsured Services ARE NOT covered by the Annual Block Fee:

  • Missed Appointment which are not cancelled within 24 hours

  • Extensive Disability Report / Insurance Forms including private insurance disability application forms

  • Physicals requested by a 3rd party (such examples being child's camp / school physical / driver's physical / employment physical)

  • Transfer of Records

  • Medical / Legal Reports

  • Invalid Health Card (monies will be returned once a valid Health Card is provided)

  • Photocopying medical reports at patient request

  • Faxing reports to another physician at patient request

  • Treating benign skin lesions

  • TB skin test

  • PAP test outside of OHIP screening guidelines

  • Auto accident insurance form
